Carers Information Card

Carers, families and friends can play a vital role in supporting someone who is experiencing mental health difficulties, but their own needs can sometimes be overlooked.

The Trust’s Carers Information Card was created to recognise that role and make it easier for carers to find help for themselves. Built around the message “You are supporting someone, let me support you,” the card brings together practical information, local contacts and routes to further support across Humber and North Yorkshire.

It is designed to be something that staff can easily share with carers and families, helping people understand what support is available and where to turn for advice. The card also includes a QR code linking directly to the Trust’s carers information webpage, so people can access the latest information and resources online.

When the Humber and North Yorkshire Dynamic Support Keyworker Service reviewed the card, they saw an opportunity to make it even more useful. Staff checked and updated contact details, added services from across the wider area and worked with the Trust’s designer to make the information clearer and easier to navigate.

Importantly, carers, people with lived experience and local support organisations were involved in shaping the changes. Their feedback helped ensure the card reflected what carers actually need and how they are most likely to use it.

The work also led to improvements to the Trust’s online carers information, creating a stronger link between the printed card and up-to-date digital support.

The project was recognised for its collaborative approach and received a Co-production Stamp from the Patient and Carer Experience Team.

The updated card can now be shared with carers and families across Humber and North Yorkshire, helping more people feel recognised, informed and connected to the support available to them.
